So this work seems to have been revealed by Khyentse Chokyi Lodro and offered to Dilgo Khyentse. Or, as it says elsewhere, it welled forth from the expanse of his realization ('jam dbyangs chos kyi blo gros pas dgongs klong nas phyungs) +
I'm assuming the author is Dilgo Khyentse's teacher Shechen Gyaltsab IV, but please revise if it turns out to be incorrect. +
This was likely received by Khyentse Wangpo in a vision +
Not sure if b+hu su ku refers to the Indian siddha, a.k.a. Shantideva, or if it is merely the name given to this particular practice. +
mi 'gyur dpal sgron is recorded as the "author of the source text" since she is the source from whom the author received these teachings. +
Dudjom Rinpoche mentions several other sources, including the treasures of Stag sham nus ldan rdo rje and Chos gling, from which he based this work on. +
This work was extracted and edited from the Dri med bshags rgyud (dam tshig thams cad kyi nyams chag skong ba'i lung lnga bshags pa thams cad kyi rgyud dri ma med pa'i rgyal po) found in various editions of the rnying ma rgyud 'bum. +
This work is signed only as dpal karma pa, which seems to be the Karmapa 15 based on his usage of this signature in other works. +
This work is drawn from a variety of treasure cycles, though only a nonspecific reference to the treasures of klong gsal snying po is mentioned in the colophon. +
According to the colophon this work seems to have originally been revealed in public by Namcho Mingyur Dorje. +
Not sure which Tamdrin cycle the first is drawn from, as there doesn't seem to be a Padma Lingpa Tamdrin Treasure in the Rinchen Terdzo. Also, not sure whom the sngags 'chang mgar ston 'bum me referred to in the second colophon is either. +
This works seems to be a compilation of treasures, though only Minling Terchen and Padma Lingpa are mentioned by name in the colophons. +
I'm giving authorship credit to mnyam nyid rdo rje, though I think he's actually the author of the work that this one was based on. +
This work appears to have gone through several stages of revision. The scribe is associated with the initial source work by Minling Terchen. +

Though the Terton signs this work as o rgyan gar dbang 'chi med gling pa, I'm using his name gar dbang 'chi med rdo rje, as this is how it appears in Kongtrul's Terton Gyatsa. Furthermore, Dudjom Rinpoche refers to the same Terton as kun bzang 'od zer in his Thob yig, which certainly seems to refer to the same person. However, the margins of the current printing state "chos gling tshe sgrub.... ", which in the RTZ is an abbreviation usually reserved for Chos rje gling pa, whom the Terton seems to have some connection to based upon Kongtrul's bio in the Terton Gyatsa. +
This work draws liturgical materials from tshe 'gugs and g.yang 'gugs practices from the cycle rtsa gsum rig 'dzin srog sgrub revealed by Lha btsun nam mkha' 'jigs med, which is why he is recorded as the source revealer. +
I'm basing authorship of these works on Balpo's treasures, which were rediscovered by Khyentse Wangpo, on Kongtrul's claims in the Terton Gyatsa that he composed the related material. +
I am presuming this was written by the second Dzaka Tulku, Kunzang Namgyal, based on similar works that he has written. +
